ALTA owns the copyright on all forms used for title insurance in the United States. Holding a Policy Forms License with ALTA is a requirement of all issuing agents of title insurance to comply with this copyright. It grants them the continued use of ALTA’s Policy Forms for the calendar year.
The Policy Forms License Fee Waiver is submitted when you/your company issued 50 or less title policies in the previous year. The waiver waives the license fee for the current year.
- Log onto http://ALTA.org and click on the Store at the top to the left of MY ACCOUNT. Click on Policy Forms License
- Click on Apply Now under Policy Forms License Fee Waiver **** this is where you can print your certificate as well.
- If you have more than one company, you are contact for be sure to pick the company you want the waiver to go.

Confirm information is correct. If you click on Update Information it is only updating your personal information. Any updates to company information please email licensing@alta.org.

Here you will need to certify you issued 50 or less and then select who your Underwriter(s)
- Scroll down – Be sure to check the box if you want to receive a printed certificate in the mail. Lastly, click on Apply for Occasional Use Waiver. ALTA will receive your waiver request and process accordingly.
